The forms cannot be completed online as your doctor’s signature and surgery details are required.
Complete both forms and send them to the contact address.
The Trustees of CCTA will consider your application as soon as they can, contacting you to let you know their decision quickly and by your preferred method
The nature of the support offered will depend on the adventure you have chosen, the availability of funds and resources and the timescale.
